Description SUMMARY: Provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative or preventative nursing care and is accountable for making decisions that are based on the individual’s nursing experience and educational preparation. Position Summary Expanded: Under the Oklahoma Medical Marijuana and Patient Protection Act, (“The Unity Bill”) this position is considered a “Safety-sensitive position. “Safety-sensitive” is defined to include “any job that includes tasks or duties that could affect the safety and health of the employee performing the task or others.” Employees working in “safety-sensitive” roles are subject to this exception and subject to disciplinary action in the event of a positive test for marijuana or its metabolites. Requirements RESPNSIBILITIES: · Provide nursing care in accordance with the client’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care. · Assist the Registered Nurse in carrying out the plan of care. Provides skilled nursing services in accordance with the orders of a physician and under the plan of care in patient’s home. Responsible for the observation, assessment nursing diagnosis, counsel, care and health teaching of the ill, injured or confined and the maintenance of health and prevention of illness. · Records the health needs of the patient. · Coordinates the care of all patients in his/her case load. · Makes referrals to other agencies or services as needed. · Participates in in-service programs, staff meetings and weekly Case Conference meetings. · Complies with Home Health Compliance Plan, Infection Control and HIPPA policies. · Available to work after hours and weekends on an on-call basis, per policy to meet the client car requirements. Minimum Qualifications: · Licensed Practical Nurse in the State of Oklahoma · Home Health Care experience preferred · Excellent communication and time-management skills with ability to work independently · Knowledge of regulatory standards and compliance · Working Knowledge of computer and software applications used in job function (Word, Excel, Outlook and EMR software) · Current Automobile insurance coverage · Workable knowledge of Medicare, CHAP accreditation criteria and standards · Willing to be on call as assigned Education and/or Experience: High School Diploma or Equivalent Certificates, Licenses, Registrations: ACLS, CPR and Licensed Practical Nurse License for the State of Oklahoma ESSENTIAL FUNCTIONS AND RESPONSIBILITIES To perform this job successfully, an individual must be able to perform each key function satisfactorily based on five overall skills categories; assessment, planning, intervention, evaluation and teaching/coaching. Responsibilities
