QUALIFICATIONS

• Associate’s Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
• 3 Years Acute Care or Long-Term Care Experience - Required
• 1 year Home Health Experience - Preferred
• Licensed Practical Nurse (LPN)- State Licensing Board - Required
• Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
• Driver’s License - Required
• Auto Insurance - Required
