Growing regional hospice agency is looking for seasoned Hospice Sales/Marketing professional (Transitional Care Specialist) in the Cottonwood/Verde Valley area.

Transitional Care Specialists:

• Generate appropriate patient referrals from customers and promote growth of referrals over time.
• Represent the agency in the community and its services in a competent, professional manner consistent with the organizations’ service and teamwork culture.
• Establish and maintain professional relationships with key physicians, senior living communities, skilled nursing facilities and hospital personnel.
• Explain our hospice services to patients and families in a manner that results in the admission of appropriate patients.
• Interact with patients, families, and referral sources.
• Oversee pending patients and routinely follow the progress of prospective patients.
• Work closely with patient care in transitioning patients as they are admitted to our services
• Develop and present to leadership community growth strategies that will position the agency as the hospice and employer of choice and increase our ability to care for more patients and families. Implement strategies that are approved.
• Manage related expenditures in a fiscally responsible manner in accordance with the Company’s budget.
