What You’ll Do Participate as an active member of the interdisciplinary team, attending assessments and daily team meetings to share updates and advocate for participants. Develop personalized care plans after assessing participant needs for in-home care provided by Home Care Aides and/or Specialists. Deliver skilled nursing visits under physician orders. Respond to participant and family concerns with compassion and professionalism. Contribute to quality improvement initiatives and maintain accurate documentation. Requirements What We’re Looking For Licensure: Current RN or LPN license in the State of Oklahoma. Experience: Minimum 2 years of nursing experience (wound care preferred) and at least 1 year working with frail or elderly populations. Skills: Strong assessment abilities, organizational skills, and the ability to collaborate across diverse teams. Other: Valid driver’s license, proof of insurance, reliable personal vehicle, and a clean driving record.
