Key Responsibilities Perform in-home assessments to determine functional impairments and therapy needs Develop and implement individualized plans of care that support safety, self-care, and mobility Provide hands-on interventions focused on ADLs, home safety, and patient-specific goals Educate patients and caregivers in the use of adaptive equipment and techniques Collaborate with physical therapists, nurses, aides, and physicians for coordinated care Document progress and modify care plans in response to clinical changes Support safe transitions and long-term functional wellness Requirements What We’re Looking For Licensed Occupational Therapist (OTR/L) in the state of New Mexico Minimum of 1 year of clinical experience (home health experience preferred) Strong problem-solving, communication, and time management skills Comfort working independently in patients’ homes Experience with EMR systems and mobile charting tools Passion for enabling independence and dignity through skilled therapy Alignment with our mission: to bring out the courage in others Why Join Us At Corus Home Health, we are therapists — and we lead like it.
