As a Stonerise Home Health Physical Therapist (PT), you will be responsible for providing physical therapy services and are responsible for the supervision of corresponding team members. Some of your primary responsibilities include: Providing physical therapy services to patients according to the patient plan of care which may include assessing and evaluating therapeutic/rehabilitative/functional status, assessing for muscle strength, mobility, gait, ROM, etc. Initiating physical therapy programs and instructing other team members and/or families/caregivers in certain phases of the therapy with which they may work with the patient Training patients on the use of prosthetic devices Identifying patient and family/caregiver needs for other home health services and referring as necessary Preparing and submitting clinical and progress summaries based on patient goals Participating in discharge planning of patients Completing a 30-day functional reassessment and revising plan and goals accordingly Providing physical therapy consultation to home families/caregivers as needed Supervising and directing physical therapy assistants (PTA) Benefits You Will Enjoy Full-time team member benefits include: Paid Time Off Insurance benefit package (medical, dental, vision, life, and other voluntary group programs) 401 (K) retirement savings plan Tuition reimbursement Advancement opportunities Training, development & continuing education opportunities Stonerise Chaplain support Employee Assistance Program (EAP) Schedule Non-Exempt Education, Licensure & Certification Requirements Possesses a baccalaureate level degree or higher in physical therapy by an accredited organization Licensed to practice as a Physical Therapist within the state At least two (2) years of experience as a PT. Responsibilities

As a Home Health Physical Therapist (PT), you will provide physical therapy services according to patient care plans and supervise corresponding team members. You will assess patients' therapeutic needs, initiate therapy programs, and train patients and caregivers.
