Job Details Job Location: Cullman Regional Medical Center - Cullman, AL 35056 Position Type: Full Time Job Shift: Day Job Summary: Evaluate patients; develop a plan of care; provide treatment to patients; and perform OASIS assessments, case conferences, and summaries of care at 30 days, at 60 days and at discharge. Act in the role of lead supervisor for LPTA and OT staff. Assist in maintaining staff education with regard to functional assessments, safety and fall prevention. Complete documentation according to agency policy. Work with OT and scheduler to schedule 11-13 visit assessments, 17-19 visit reassessments and 30 reassessments. Demonstrate and encourage team behavior and exceptional patient/guest experiences. Uphold and promote patient safety and quality. Qualifications Education: Minimum of bachelor's degree in physical therapy. May also have a master's or PhD in physical therapy. Licensed in the state of Alabama. Experience: Minimum 2 years of experience as a physical therapist. Preferred experience in home care. Supervisory experience preferred. Additional Skills/Abilities: Should possess good leadership skills, great communication skills, critical thinking skills, flexibility, and the ability to multitask and prioritize. Must be organized and driven to complete work independently.
