JOB DESCRIPTION

The Home Quality Assurance Coordinator is responsible for the review of clinical documentation for Home Health in accordance with attending physician orders and plans of care to provide the highest quality of patient care and maintain compliance with Medicare, Medicaid, accrediting bodies and other federal and state rules and regulations.

REQUIREMENTS

  • Must have at least 2 years’ experience in home health QA review.
  • Proficiency in EMR: Kinnser or WellSky, preferred.
  • Knowledge in ICD 10, preferred.
  • Registered Nurse or Licensed Practical Nurse, preferred.
Responsibilities
  • Performs tasks in EMR QA Manager.
  • Reviews OASIS and all clinical documentations on a daily basis, ensures compliance to Medicare guidelines
  • Audit OASIS for omissions and discrepancies.
  • Ensures all required goals to meet Plan of Care are included and have required interventions on 485s and physician orders.
  • Generates the Plan of Care and physician orders to be sent to clinicians.
  • Maintains daily coordination and facilitates communication with clinicians for OASIS compliance Communicates with Home Health Director of Nursing for further clinical education on an individual clinician basis and/or team basis for any documentation trends identified.
  • Prepares and assists with Medicare pre-claim review process.
  • Assists with orientation and ongoing training of all employees regarding documentation as needed.
  • Consults with the physicians when necessary to ensure and maintain safe, efficient, and quality patient care.
