Description As a Home Health Registered Nurse (RN), you will provide compassionate and comprehensive nursing care to patients in their homes. Your role will involve assessing, planning, implementing, and evaluating patient care in collaboration with physicians, caregivers, and other healthcare professionals. You will be responsible for managing a diverse caseload of patients with various medical conditions, ensuring the highest standards of care are maintained. Schedule: Full-Time, Mon - Fri, Day Shift Primary Responsibilities and Duties: Conduct comprehensive patient assessments and develop individualized care plans based on patient needs and medical history. Administer medications and treatments as prescribed by physicians. Monitor and document patient progress, changes in condition, and response to treatments. Educate patients and their families on disease processes, medications, and home care techniques. Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care. Provide wound care, catheter care, and other specialized nursing procedures as required. Perform health screenings and diagnostic tests, including taking vital signs and managing chronic conditions. Ensure compliance with healthcare regulations, protocols, and best practices. Maintain accurate and up to date patient records and documentation. Offer emotional support and guidance to patients and families throughout the care process. Perform all other duties assigned. Requirements Job Qualifications: Valid license as a Registered Nurse (RN) in KS and MO. Some nursing experience, preferably in a home health or similar setting. Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care. Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals. Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment. Understanding of regulatory requirements governing nursing practices and home health agencies. Ability to build rapport with patients through a compassionate and caring demeanor. Physical Requirements: Ability to lift and transfer patients, with or without assistance, up to 50lbs. Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. Strength to handle medical equipment and assist patients with mobility and physical activities. Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling delicate medical instruments. Ability to respond swiftly and efficiently to emergency situations. Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager. Job requires accepting criticism and dealing calmly and effectively with high-stress situations. The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing. The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.
