Description A Home Health Registered Nurse (RN) will deliver comprehensive, patient-centered care in a home setting, managing patients’ medical needs and promoting overall health and well-being. This role requires strong clinical skills, excellent communication abilities, and the capability to work independently while collaborating with an interdisciplinary healthcare team. Schedule: PRN, Weekends, Weekdays, On-Call Primary Responsibilities and Duties: Administer medications and treatments as prescribed by physicians and in accordance with home health care protocols. Perform routine and specialized nursing tasks, including wound care, catheter insertion, and monitoring vital signs. Assist in developing and implementing individualized care plans in collaboration with the patient, family, and healthcare team. Monitor and document patient progress, response to treatments, and any changes in condition, reporting significant findings. Provide education and support to patients and their families on disease management, medication use, and home care techniques. Assist with patient mobility, transfers, and activities of daily living as needed, ensuring safety and comfort. Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards. Collaborate with LPNs, physicians, and other healthcare professionals to ensure coordinated and comprehensive care. Respond to patient needs and changes in condition promptly, providing appropriate interventions and support. Adhere to infection control and safety procedures to protect patients and staff. Perform all other duties assigned. Requirements Qualifications: Graduate of an approved school of professional nursing who is licensed as a Registered Nurse by the state in which practicing. Minimum of two (2) years nursing experience in Home Health required. Ability to supervise and direct members of client care team. Demonstrated strong and effective written and verbal communication skills and good interpersonal skills. Ability to deal tactfully with clients and the community. Demonstrate organizational and time management skills. Licensed driver with automobile insurance. Physical Demands: Requires a full range of body motion including the handling and lifting of patients. Manual and finger dexterity and eye-hand coordination required. Occasionally will lift and carry items weighing up to 50 pounds or greater. Requires corrected vision and hearing to normal range. May require working under stressful conditions or working irregular hours. Involves exposure to communicable diseases. Requires a current driver’s license and the ability to drive and access a patient’s home. The majority of work is performed in providing patient care services in the patient’s home. Exposure to communicable diseases is common.
