This is your opportunity to focus on quality care. Responsibilities: Deliver high-quality, individualized nursing care to patients in their homes as prescribed by the physician. Collaborate with an interdisciplinary care team to develop, implement, and adjust the patient’s plan of care. Monitor and evaluate patient progress, reporting findings to the physician, case manager, and family as appropriate. Administer treatments and manage nursing interventions based on clinical assessments. Participate in on-call rotation (approximately every 3–4 weeks) and perform PRN visits as needed. Complete documentation using Kinnser (WellSky) system. Other duties as assigned.
