Description We are seeking a dedicated and compassionate full-time Registered Nurse Case Manager to join our healthcare team. In this rewarding role, you will coordinate and oversee the individualized care of patients, ensuring they receive the highest standard of medical services in their home environment. Schedule: Full-Time, Weekdays (M-F), On-Call, 8 hour week days Key Responsibilities: Coordinate and manage the comprehensive care of assigned patients. Develop, implement, and regularly update individualized care plans. Perform skilled nursing assessments and provide direct patient care. Collaborate with physicians, therapists, social workers, and other healthcare professionals. Educate patients and families on disease processes, medications, treatments, and care plans. Ensure timely and accurate documentation of all care activities. Participate in case conferences and interdisciplinary team meetings. Serve as a compassionate resource for patients and families, providing emotional support and guidance. Requirements Active Registered Nurse (RN) license in the state of Pennsylvania. Previous experience in home health, hospice, or case management preferred. Strong clinical assessment and critical thinking skills. Excellent communication, organizational, and time-management abilities. Compassionate, patient-centered approach. Valid driver’s license, reliable transportation, and current auto insurance.
