As a Stonerise Home Health RN, you will plan and direct services to meet the needs of the patients and families. You will develop a working patient care plan and provide direction/supervision to paraprofessional nursing staff. Some of your primary responsibilities include: Completing initial assessments of patients and families to determine home care needs. Providing a complete physical assessment and history of current/previous illness(es) Developing a care plan, which establishes goals based on diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
