Key Responsibilities: Receive and process home health referrals via phone, fax, email, and hand delivery Verify insurance benefits and obtain authorizations from payers Accurately input patient data and visit details into the EMR and mileage tracking system Coordinate with the clinical team to schedule admissions and follow-up care Contact patients to confirm start-of-care dates and assign staff accordingly Track and update the daily admissions list, maintaining clear documentation Ensure timely communication with physicians for order sign-off and Face-to-Face documentation Collaborate across departments to support high-quality patient transitions Requirements What We’re Looking For: 1+ year of experience in healthcare scheduling or referral coordination preferred Bilingual and/or experience working with diverse populations is a plus Calm under pressure and highly detail-oriented Familiar with EMR systems and Microsoft Office Suite Excellent verbal and written communication skills Comfortable working independently and as part of a dynamic team Reliable transportation and ability to travel locally if needed Enthusiastic about learning and growing into leadership opportunities Why Join Us: At Corus, you’re more than your job title — you’re part of a mission-driven team that strives “to bring out the courage in others.” We offer a collaborative, uplifting culture where your contributions are valued and your growth is supported.
