Our model allows patients to receive high-quality care from the comfort of their homes while staying connected to a supportive care team. We are looking for a detail-oriented, patient-focused coordinator to play a key role in guiding patients through their sleep testing and treatment journey. Position Summary The Home Sleep Test Treatment Coordinator is responsible for ensuring a smooth, organized, and supportive patient experience from initial testing through follow-up. This role is ideal for someone who enjoys coordination, patient communication, and problem-solving in a fast-paced healthcare environment. Key Responsibilities Coordinate the distribution and return of Home Sleep Tests to patients Follow up with patients to support therapy and encourage treatment completion Troubleshoot patient questions and issues related to sleep testing Monitor patient compliance and test success, ensuring timely follow-up Respond to patient calls and provide ongoing support throughout the process Maintain accurate and up-to-date information in patient care platforms Track and report key metrics and patient progress on a weekly basis Collaborate with providers and team members to ensure continuity and quality of care Requirements Strong organizational and logistical coordination skills Excellent communication and follow-up abilities with a patient-first mindset Comfortable navigating multiple systems and managing competing priorities Detail-oriented with the ability to work independently Proactive problem-solver who takes ownership of tasks Preferred (but not required): Experience in a medical or healthcare setting Exposure to sleep medicine or pulmonary care Benefits Health, dental, and vision insurance 401(k) plan Paid Time Off (PTO) Why Join Frontier Sleep and Wellness?
