Norco, Inc logo Home Transition Coordinator 4211 SE International Way, Milwaukie, OR 97222   Job Description  Norco is currently seeking an individual to fill our Home Transition Coordinator position. The Home Transition Coordinator is responsible for meeting response times for patient discharge from the healthcare facilities and reflecting Norco’s image with the provider community and be an extension of customer service. Problem solving includes continual prioritization and re-prioritization of customer (patient and referral source) needs. Impact includes potential for significant customer service implications. The following essential duties may be carried out with or without reasonable accommodation:    * Communicate with referral sources as an extension of the Sales team  * Work in harmony with outside sales teams in order to meet company objectives  * Act as a liaison between Norco Medical and the patient/client, family, care giver, physician, and other healthcare agencies and staff on the case  * Deliver patients’ durable medical goods (DME) to facilities  * Conduct, participate in, and attend educational meetings and in-services for healthcare workers, patients/clients, families, care givers, physicians, and community members  * Assist Medical Customer Service Representatives in arranging necessary referral to assist in continuity of care for the patient community  * Assist Medical Customer Service in researching and pricing special order items  * Assume full responsibility for their own actions, professional skills, and attitudes  * Keep cognizant of third-party reimbursement, criteria services, and equipment offered, billing and patient/client paperwork  * Adhere to all established procedures, policies, and safety rules  * Greet customers in a professional and enthusiastic way, consistently demonstrate empathy and patience with customers  * Demonstrate in-depth knowledge of products, placement, availability, price, and use  * Efficiently assess customer needs for product or services  * Educate patients/clients on medical products and services  * Fit and sell medical products and services to customers  * Keep apprised of industry billing criteria as it relates to specific customer insurance requirements  * Maintain an understanding of Customer Service and equipment set-up processes, and a good understanding of the billing process  * Process customer orders and accurately input customer orders into the computer  * Initiate and complete new patient referral information files and assist in setting up new accounts where applicable  * Route patient information to appropriate personnel/departments as necessary to order, deliver and bill medical products and services  * Follow safe work practices in performance of all duties and follow the company’s safety policies daily  * Perform other tasks as assigned or needed  * Stay informed of and adhere to company policies and procedures  * Work well with a team  * Support Norco Medical by being loyal and informing the manager of patient/referral concerns and problems to be resolved  * Answer billing and other customer questions or direct customer to proper billing personnel  * Participate in both vendor and Norco in-services  * Keep work area clean and safe    Qualifications  * High school diploma or equivalent, college degree in business or marketing preferred  * One year home care experience preferred  * Sales and marketing experience preferred  * Valid regular driver’s license  * Possess excellent “people skills” and a genuine desire to help others  * Ability to effectively communicate verbally and in writing  * Perform basic mathematics calculations: add, subtract, multiply, divide  * Ability to work with diverse clientele  * In-depth knowledge of home medical products, placement, availability, and pricing beneficial  * Accurately enter data into computer and acquire a working knowledge or any software applications applicable to the position  * Detail oriented  * Ability to adapt to rapidly changing priorities  * Excellent organizational and prioritization capabilities  * Ability to work in frequently high stress situations    Additional Information  Norco offers a competitive compensation/benefit package, including:    * Employee Stock Ownership Plan (ESOP)  * Health, Vision and Dental Insurance  * Health Savings Account (HSA)   * Medical and Dependent Care Flex Accounts (FSA)  * Life Insurance provided at no cost to employee by Norco through United Heritage  * Supplemental Accident, Disease, and Life Insurance through Colonial Life Insurance  * Employee Tuition Reimbursement  * 401(k) with Employer Matching  * Employee Discount on products sold by Norco    Company Description    Founded in 1948, Norco is headquartered in Boise, Idaho and has more than 70 branches in Idaho, Montana, Oregon, Nevada, Washington, Utah and Wyoming.
