HEALTHY FAMILIES HOME VISITATION WORKER

We are seeking to fill a full-time tempoary position with our team! The Healthy Families Home Visitation Worker/Parent Education Facilitator will work within the Healthy Families Team (28 hours/week Home Visitation and 7 hours a week Parent Ed). In this role, accountability focuses on providing family-centered, strength-based interventions directed at establishing a trusting relationship within families to optimize parent/child relationships, increase parental capacity and strengthen infant, child and parental resiliency and well-being.

Duties and Responsibilities

  • Support parents/ caregivers to improve the family support system, increase the family’s ability to problem solve and assume the role of advocate for themselves and their children.
  • Support parents/caregivers to assess and monitor the development of their child.
  • Plan and facilitate age-appropriate interaction between parent and child for scheduled visits.
  • Support the development of the Individual Family Support Plan based on family’s strengths, stresses, and needs in partnership with the family in required time frames.
  • Complete necessary screens with families ie: Child development, Family Violence, etc.
  • Identify and maintain knowledge of outside referral sources and providers.

· Community networking – build and maintain relationships with local community agencies.
· Ensure that required documentation and data collection is completed within time frames.
· Ability to work independently and within a team.
· Participate in community events and promotion of programs/organization
· Participate in internal and external committees
Attend training, both as required by the position(s) listed as well as partake in other opportunities for professional development.
· Understand and practice with the understanding in the belief that children need to be nurtured and maintain the importance of attachment and the importance of secure relationships.
· All other duties as identified/requested by Supervisor(s). Staff reports directly to Healthy Families Team Lead.

QUALIFICATIONS:

· Diploma or degree in Human services, Social Work or Early Childhood Development or Minimum of 3 years’ experience working with families
· Ability to establish a trusting relationship quickly with individuals
· Respect for the diversity of cultural difference and beliefs
· Work from a strength-based, trauma informed approach
· Knowledge of community resources
· Proficiency in Microsoft office
· Experience working with spreadsheets and data bases
· Experience with outcome reporting is an asset
· Excellent interpersonal skills, communication and written skills
· Excellent verbal and written communication skills
· Proven organizational ability with the capability to multi-task
· Ability to handle change and transition in a fast-past environment
· Knowledge of the non-profit/charitable sector is an asset
· A valid driver’s license and access to reliable vehicle
· Satisfactory background screening, including a criminal record/police information check and child welfare check.
· Maintain current Crim Check, Intervention check and First Aid certificates as well as valid drivers’ licence and reliable transportation, including minimum $2,000,000.00 liability.
· Ability to flex work schedule to accommodate families.
· Build trusting relationships with mothers, fathers, extended family, and caregivers
· Be respectful and sensitive to the needs and rights of others, including those with differing cultural, racial, sexual or ethnic identities.
· Clear communication with other staff members, supervisors and families.
· Practice ethical and professional boundaries.
· Punctuality and accountability.
· Follow policies, procedures and practices of the organizations
