SUMMARY:   The RN provides expert professional nursing care and primarily delivers care to patients in their own homes as part of a value-based care program. The RN assesses, plans, implements, and evaluates care for patients with urgent, emergent, or potential health problems, with a focus on improving clinical outcomes, care coordination, and patient experience while reducing unnecessary utilization. The RN collaborates with interdisciplinary teams to support chronic disease management, care transitions, preventive care, and population health initiatives, and participates in the management of the department. PREFERRED QUALIFICATIONS: Formal training that will be indicated by a graduation certificate from an accredited RN school of nursing; current state licensure required; BLS certification; experience in clinic-based nursing; dedication to patient satisfaction; ability to communicate effectively; and the ability to work compatibly with physicians and other staff members. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to make home visits to patients/families in a variety of settings. PHYSICAL GUIDELINES:  Physical guidelines include the ability to move, traverse, position self, remain in a stationary position and negotiate steps for up to eight hours per day, lift, move and transport patients and materials; manual dexterity and motor coordination; articulate speech; read and understand medical information; visual acuity and ability to distinguish colors.  NOTE: This document is intended to describe the general nature and level of work performed.
