Description POSITION SUMMARY: This position has a dual role, with half the day spent in the office and the other half in the field. The Home Care Scheduling Assistant is responsible for coordinating and managing schedules for LIFE PACE Home Care participants and aide staff. The HomeCare CNA/Specialist is primarily responsible for helping assist with tasks including, but not limited to light housekeeping, companionship, meal preparation, running errands, and shopping. Encouraging independence and involvement of the client while performing assigned tasks. PRINCIPAL DUTIES AND RESPONSIBILITIES: The following represents the majority of the duties performed by the position, but is not meant to be all-inclusive nor prevent other duties from being assigned as needed. Scheduling Assistant: Understands and adheres to established LIFE PACE Home Care policies and procedures. Creates and maintains staff schedules. Provides staffing for sick leave, vacation, long term leave. Schedules client appointments/visits according to care plans and staff availability. Enters scheduling data and create schedules. Contact care providers and clients regarding day-to-day changes. Discerns client services required as outlined in agreements, urgent requests and care plans. Enters staff and client information into database. Maintains staff and client database (e.g. CQA, dates and reminders of TB, client admission, client hospital admission and discharge). Maintains staff and client statistics, and reports. Maintains call – in and retention report. Performs general office duties including but not limited to word processing, photocopies, files, shreds, sort/distributes mail, provides reception and telephone services. Other duties as required. On Call when necessary. HomeCare CNA/Specialist Provides home care services in Participants’ home. This may include sweeping, mopping, vacuuming, making a bed, laundry, cleaning the kitchen, cleaning the bathroom and other light housekeeping duties in their home. Running errands for Participants while they are at the Participant’s home. Completes and maintains accurate records of the participants’ attendance, activity participation, nutritional intake (as required), health monitoring, and incidents in the participant’s home. Maintains a safe secure environment. Follow all body mechanics standards, ensuring that staff and participants remain injury free. Report all safety issues to the HomeCare Supervisor and monitor the environment for the proper functioning of all equipment. Work cooperatively with all staff. Foster working relationships that facilitate teamwork, mentoring and a positive work environment. Follow all LIFE policies and procedures and practice guidelines. Requirements CERTIFICATION, LICENSES, REGISTRATIONS: · Must have a valid Oklahoma driver’s license good driving record and car insurance. · Upon hire, will obtain/maintain current certifications in CPR and First Aid EDUCATION AND EXPERIENCE: · High school diploma or GED. · A minimum of one-year experience working with the elderly population preferred. KNOWLEDGE, SKILLS, and ABILITIES: · Computer skills including but not limited to MS Office, MS Excel and Scheduling program. · Knowledge of medical terminology required. · Interpersonal, organizational and communication skills. · Ability to carry out directions, read and write. · Maturity and ability to deal effectively with the demands of the job. · Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the program. Attends and successfully completes mandatory HIPAA/HITECH training as required. · Ability to work under pressure to meet deadlines, handle several tasks simultaneously. · Ability to follow directions and work independently with minimal supervision. · Must have excellent communication skills, be able to cope with mental and emotional stress related to the position, have flexibility and personal integrity.
Responsibilities
