ABOUT THE ROLE
As our Homelessness Engagement Practitioner, you will play a critical role in supporting vulnerable families, young people (aged 16 – 24) and individuals at risk of homelessness. You’ll assess your clients’ living circumstances and create case plans to secure them more suitable, long-term housing. Your case management will balance between early intervention, crisis and post-crisis support depending on the needs of your clients.
You will also work collaboratively with community partners and facilitate connections between clients and the wider community.
