The HopeLine – Care Coordinator Description/Expectations/Principles - Hopeline Continued Care for Montgomery County, Ohio The HopeLine position is a 40-hour per week job that is centered around outreach to clients in active substance abuse in the community in which they live.   Job Description  * Respectfully work one-on-one with a person who is in active or recent substance abuse. * Engage and build relationships with clients over time via face-to face, phone, and electronic messaging. * When needed, quickly secure viable treatment options for a client based on fit,   treatment availability and admission requirements/insurance coverage. This can also include transporting a client to a facility if needed. * Build collaborative relationships with treatment providers. * Understand and leverage community wrap-around services to help clients with day- to-day issues that can de-rail recovery. * Ensure that referral and linkages are made for the entire family when appropriate.   Team Operating Principles 1. Individual Expectations 1. Ability to respectfully assess a person to offer appropriate and unbiased recovery service referrals. 2. Ability to work independently demonstrating initiative, manage own schedule, and ability to be very fluid based on priorities and emergencies that surface on any given day. 3. Ability to reach out and develop working relationships with medical facilities, public health departments, local government agencies, law enforcement, treatment providers, and emergency rooms/responders.  4. Where applicable, submit referrals to the Hopeline Continuing Care team based on the current work process and requirements.  5. Have a minimum of 25 interactions with individual clients a week.  6. Enter accurate records by documenting client interaction notes in CorData within 7 days of the interaction. 7. By the 3rd of the month, submit all mileage tracking for the previous month.  8. Maintain confidentiality of client records and other sensitive subject matter. 9. If a supervisor or team leader communicates to you via text or phone call, communicate back at a minimum by the end of day.  10. Meet all training and employee requirements as a Beckett Springs employee.  Qualifications Education: Bachelor’s degree in marketing, business administration or related field preferred, combination of education and professional experience may be acceptable Experience: Previous experience in business development at psychiatric healthcare facility preferred License: Valid driver’s license and a favorable Motor Vehicle Report (MVR), along with proof of insurance when using own auto.
