ABOUT THE ROLE

This is a meaningful leadership opportunity within Grampians Health’s Mental Health and Wellbeing Services, supporting individuals at significant risk of suicide. The HOPE Program (Hospital Outreach Post-suicidal Engagement) provides clinical and psychosocial outreach for individuals following a suicide attempt or experiencing serious suicidal ideation.
As Team Leader, you’ll lead a multidisciplinary team that works with clients and their support networks to strengthen protective factors and reduce risk. You’ll play a vital role in ensuring service quality, accessibility, and alignment with strategic reform across the Grampians region.

KEY RESPONSIBILITIES

  • Lead and oversee daily operations of the HOPE team
  • Coordinate outreach services and client care pathways
  • Supervise and support clinical team members
  • Promote service quality, safety and client-centred care
  • Contribute to service development and reform initiatives
  • Liaise with internal stakeholders and community supports
