JOB SUMMARY:

• The primary function is to generate and manage leads from various referral sources while liaising between the Agency and the community regarding community and customer needs.
• Help identify new patients in need of home health services.
• Develops and Maintains relationships with payors and community referral sources/organizations to achieve referral and admission volume.
• Must grow and maintain a pipeline of patient referral sources to increase the agency’s overall patient census consistently.

• Job Type: Hospice, Geriatrics
• Work Hours: Full-time and on the road
• Compensation: Commission Pay. Mileage reimbursement
