Description The Admissions Visit Nurse is responsible for assuring that Adoray Home Health and Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a patient’s home or in a health care facility. The Admission Visit nurse will work in coordination with the Registered Nurse Case Manager to provide hospice nursing care in accordance with the plan of care and physician orders. The RN completes all necessary documentation and participates in other duties as assigned. ESSENTIAL DUTIES AND RESPONSIBILITIES · Provides information, pre-assessment, and admission services to patients who have been referred to Adoray, conducting pre-assessment visits in the home or health care facility as necessary. · Communicates with the Intake Department, the referring physician and Hospice Medical Director to assure appropriateness of each referral and those necessary certifications are obtained. · Completes comprehensive assessment within 5 days of patient admission. · Develops accurate and individualized POC at the time of admission and ongoing. · Ensures all hospice admission documentation is completed at time of admission. · Develops appropriate care plans for the patient’s overall condition at the time of admission. · Keeps the Intake Department informed about progress of referral/admission. · Makes appropriate referrals for other discipline services and medical equipment/supply companies to meet patient care needs in a timely manner. Coordinates care of patients with a variety of disciplines and community resources, both internal and external in a fiscally responsible manner. · Completes and submits documentation within agency timeliness policies that are complete, legible, and clearly reflect the care given. · Provides assigned nursing visits to patients as directed. · Completes rotating on call shifts as assigned. · Create, follow, and revise an individualized plan of care as directed by the RN case manager on an ongoing basis. · Performs specialized procedures as needed or assigned. · Ensure physicians orders are carried out in a timely manner as assigned. · Provides education and teaching to patients and/or families. · Provides medication reconciliation, education, monitors for appropriateness, side effects, effectiveness, and adherence to medication regimen at each home visit. · Assist patients in learning appropriate self-care techniques. · Prepares equipment and materials for treatment, observing aseptic technique as required. · Ensures timely and accurate communication of pertinent information to facility staff, caregivers, and interdisciplinary care team. · Complies with universal precautions and standard infection control protocols and general safety policies and procedures. · Maintains accurate, complete documentation of nursing activities on medical record. · Ensures documentation is completed in the patient’s medical record at point of care. · Updates schedule daily. · Provides appropriate agency staff with notification of visits made and availability. · Maintains close communications with RN case manager regarding general observations of patient and patient/family status. · Communicates with RN case manager regarding observations in which additional service may be of benefit as appropriate. · Promptly informs the RN case manager, hospice NP, physician or other persons participating in the patients care of changes in the condition and needs. · Provides input to RN case manager regarding relevant information needed for the patient care CIT meetings. · Completes hospice discharge summaries at time of death. · Other duties as assigned. Minimum Qualifications · BSN or minimum of two years of hospice care experienced preferred. · Minimum of two years of clinical experience. · Current Wisconsin RN license required. · Current driver's license. · Able to take on call responsibilities on rotating basis. · Excellent communication skills.
