POSITION SUMMARY:

The Hospice Admission Nurse Coordinator is responsible for managing and overseeing the intake process for new patients, ensuring a smooth transition into hospice care. This position involves conducting patient assessments, collaborating with healthcare providers, and ensuring compliance with regulatory requirements, while providing compassionate care and support to patients and their families.

QUALIFICATIONS:

  • Registered Nurse (RN) with an active state license.
  • Minimum of 2 years of nursing experience, preferably in hospice, palliative care, or home health.
  • Knowledge of hospice regulations and admission procedures.
  • Strong communication and interpersonal skills, with the ability to work collaboratively within a multidisciplinary team.
  • Compassionate and empathetic approach to patient care.
  • Excellent organizational and time management skills.
  • Ability to handle sensitive situations with professionalism and integrity.
  • BLS/CPR certification required; Hospice certification (CHPN) preferred.
Responsibilities
  • Conduct comprehensive assessments of patients referred for hospice services, including physical, emotional, and spiritual needs.
  • Complete necessary documentation for patient admissions, including medical records and authorization forms.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Coordinate patient admissions with physicians, case managers, and family members.
  • Ensure all necessary medical and insurance paperwork is completed accurately and on time.
  • Educate patients and families about hospice services, the hospice care process, and options available.
  • Serve as a liaison between patients, families, and healthcare providers.
  • Maintain accurate and up-to-date patient records in compliance with regulations and organizational policies.
  • Participate in on-call duties and assist with after-hours admissions as needed.
