Description Hospice Admissions Liaison & Marketer (Sales) (Hospice Experience Required) Supervised By: Executive Director Interrelationships: Referral Sources, Physicians, Facilities, IDG Team, Patients & Families Job Summary Tender Care Hospice is seeking an experienced Hospice Admissions Liaison & Marketer (Sales) to drive sustained census growth through strategic referral development and high-performance sales execution. This role is designed for a true hospice sales professional, not a community event planner, who can build, own, and expand referral relationships that consistently produce admissions. The ideal candidate will demonstrate a proven ability to generate their own referral sources, exceed prior year admissions metrics, and contribute directly to monthly census growth through disciplined sales activity, relationship management, and data-driven performance. Key Expectations (This Is a Sales Role) Independently develops and maintains referral sources (physicians, hospitals, SNFs, ALFs, case managers, discharge planners, and community providers) Produces measurable monthly admissions that trend higher than last year’s performance Moves beyond passive referrals (walk-ins, cold facility calls) and demonstrate active, strategic sales planning Directly contributes to census growth, market expansion, and brand presence Primary Duties & Responsibilities Sales & Referral Development · Proactively identify, develop, and sustain new and existing referral sources that consistently generate hospice admissions. · Execute a territory sales plan designed to grow census month over month. · Establish long-term partnerships with physicians, discharge planners, hospitals, SNFs, ALFs, home health agencies, and community providers. · Track and report monthly admissions, conversion rates, referral trends, and territory performance. Hospice Expertise & Education · Educate referral partners on hospice eligibility, benefits, and Tender Care’s clinical excellence. · Serve as a hospice resource for end-of-life care planning, CMS guidelines, and disease-specific hospice criteria. · Assess referred patients for hospice eligibility in collaboration with the clinical team. Collaboration & Care Coordination · Work closely with the IDG team to ensure smooth transitions into hospice services. · Support patients and families during the referral and admission process by providing clear education and guidance. Reporting & Compliance · Maintain accurate documentation of referral activity, sales contacts, and admissions outcomes. · Ensure all sales and marketing activities comply with hospice regulations and ethical standards. Performance Metrics Monthly admissions volume Year-over-year census growth Referral source development and retention Conversion rates from referral to admission Requirements Bachelor’s degree in Marketing, Business, Healthcare Administration OR RN License (required) Minimum 2+ years of hospice sales / liaison experience (Hospice required) Documented success in meeting or exceeding hospice admissions targets Proven ability to build referral sources from the ground up Strong organizational, communication, and time-management skills Ability to work independently while collaborating with the IDG team Proficiency with CRM systems, referral tracking, and reporting metrics
