Established in 1978, we offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Hospice Program Compassionate, nonprofit end-of-life care service designed to support individuals with life-limiting illnesses and their families by focusing on comfort, dignity, and quality of life rather than curative treatment. The hospice team provides personalized pain and symptom management, emotional and spiritual support, and practical assistance wherever the patient calls home — whether that’s in their own residence, a nursing facility, or at the organization’s dedicated inpatient hospice house for more intensive needs. The program also extends bereavement support for families and works to help patients make the most of the time they have left by addressing physical, social, and emotional needs with sensitivity and respect. Hospice Admissions Nurse Plays a key role in helping patients and families begin hospice care with understanding and support. The nurse completes the initial assessment by reviewing the patient’s medical history, evaluating symptoms, and determining eligibility for hospice services. They explain the hospice philosophy, services, and care plan to the patient and family, answer questions, and obtain necessary consents. The RN admissions nurse also collaborates with physicians and the hospice care team to develop an individualized plan of care, ensures medications and equipment are arranged, and helps families feel prepared and supported during the transition to hospice care. Qualifications Required Active Registered Nurse (RN) license Graduate of an accredited nursing program (ADN or BSN) Valid driver’s license with reliable transportation Current CPR certification Strong clinical assessment and documentation skills Ability to work independently and manage time effectively Excellent communication and interpersonal skills Ability to provide compassionate care to patients and families Preferred Previous hospice, palliative care, or home health nursing experience Bachelor of Science in Nursing (BSN) Experience with admissions, care coordination, or case management Familiarity with electronic medical records (EMR) systems Knowledge of end-of-life symptom management Certification in hospice and palliative nursing (CHPN) or willingness to obtain Apply Today!
