Description About the Role As a Hospice Admissions RN at Corus Hospice, you play a pivotal role at one of the most meaningful moments in a patient and family’s journey. You are often the first clinical presence they encounter — the person who listens carefully, assesses thoughtfully, and helps families take the next step with clarity and compassion. This role blends strong clinical judgment with deep human connection. You’ll guide patients and families through hospice eligibility and enrollment, ensuring each transition is grounded in dignity, honesty, and respect. Key Responsibilities Conduct comprehensive hospice admissions assessments, including physical, emotional, psychosocial, and spiritual needs Evaluate hospice eligibility using clinical judgment, prognosis, and CMS criteria Educate patients and families on the hospice philosophy, goals of care, and available services Develop and initiate individualized plans of care in collaboration with the interdisciplinary team and attending physicians Coordinate timely admissions by completing accurate documentation and communicating across providers and settings Serve as a clinical liaison with referral sources, facilities, physicians, and internal hospice team members Advocate for patient goals, preferences, and comfort throughout the admission process Requirements What We’re Looking For Current, unencumbered RN license in New Mexico Prior hospice experience required Strong clinical judgment with the ability to make confident eligibility and admission decisions Clear, compassionate communicator with patients, families, and providers Empathetic and emotionally grounded presence in urgent or sensitive situations Highly organized and detail-oriented, with strong documentation habits Ability to work independently in the field while staying connected to a collaborative team Reliable transportation, valid driver’s license, and auto insurance Why Join Us At Corus Hospice, you’re not working alone.
