Key Responsibilities: Provide compassionate, patient-centered care in the home setting, focusing on comfort and dignity for patients with terminal illnesses. Assist with personal care tasks, such as bathing, grooming, dressing, and feeding. Monitor patient condition and report changes to the nursing team. Offer emotional support and companionship to both patients and their families. Help with light housekeeping and assist with mobility when needed. Requirements Successful completion of approved LNA training which fulfills Medicare and State regulation; Current registration by the New Hampshire Board of Nursing and evidence of twelve (12) hours of nursing related in-service education annually; Current Basic Life Support (BLS) certification; Must have successfully completed the LNA competency test with a score of 80% or better; and Possesses a valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and state minimum liability insurance.
