JOB DESCRIPTION
Park Nicollet is looking to hire a Hospice Aide, to join our Home Care/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
POSITION SUMMARY:
The Hospice Aide is a trained para-professional health care worker who is prepared to assist the professional nurse and/or other skilled professional with the patient care delivery process. The Aide also participates in the billing/charges/documentation process by adequately completing all work as assigned.
Tasks and responsibilities include
- Recognizes and reports changes in patient’s condition, including skin condition changes, or problems in the home to RN Case Manager and/or other appropriate skilled professional
- Assists patients with personal cares and activities of daily living. Cares include: bathing, hair care, oral hygiene, skin care, simple dressing changes, toileting, dressing, hearing aid care, positioning and turning, transfers, ambulation, ROM (range of motion) exercise, and other cares as assigned
- Assists patients with the use of special equipment following appropriate education. (Hoyer lift, sit to stand lift, transfer belt, whirlpool and other special bath aids. Proficient with ability to assist with wheelchair transfers, use of walkers and canes, and other commonly used aids.)
- Understands and demonstrates procedures for obtaining temperature, pulse, and respiration
- Maintains a clean and neat patient care area, changes bed linens, and performs other light housekeeping tasks as assigned
- Assists with light meal preparation. Assists with feeding following feeding competency assessment
Work Schedule: M-F 8:00am-4:30pm
REQUIRED QUALIFICATIONS:
- Education, Experience or Equivalent Combination:
- Home Health Aide or equivalent training program with certificate desired. A minimum of one year experience as a nursing assistant or home health aide if educational preparation absent
- Licensure/ Registration/ Certification:
- The Hospice Aide must successfully pass the home health aide competency written and practical tests
- Knowledge, Skills, and Abilities:
- Ability to demonstrate additional competency with medication assistance in assisted living if applicable
- The Hospice Aide must possess a current Minnesota driver’s license if travel is required and dependable transportation to get to work.
- Ability to make meaningful observations and write simple reports.
- Ability to accept and constructively use supervision.
- Ability to recognize the needs of people and to maintain good relationships in dealing with them.
- Ability to carry out simple personal care services for the patient in the home under the supervision of the RN Case Manager and/or other supervising skilled professional.
- Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
