Job Description   Provides counseling services to bereaved family members of hospice patients and the general community. Please note: The 'minimum’ rate and 'maximum’ rate refers to a 'per-visit' rate for regular assignments in this role.  Voluntary work opportunities, outside the normal scope of duty, may pay an alternate ‘per-visit' rate, depending on multiple factors (region, type of visit, discipline, etc.), and may be offered on an occasional, as-needed basis. There is an hourly rate paid for certain administrative tasks (orientation, training, meetings, etc.), which is $35.00, following completion of new hire orientation.  Job Responsibility 1.Assesses bereaved clients and provides program orientation. 2.Collaborates with healthcare team members to develop and implement  client plan of care. 3.Conducts individual, group and family counseling sessions. 4.Serves as a liaison to referral sources to facilitate continuity of care. 5.Performs related duties as required. All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act. Duties not mentioned here, but considered related are not essential functions. Job Qualification * Master's Degree required.
