Job Description Under supervision of the Hospice Team Manager and AVP Operations, act as assistant to the Hospice interdisciplinary group members and coordinate the provision of ancillary services. Job Responsibility 1.Order laboratory, x-ray, medical supplies and equipment for patients and document.  2.Accurately enters all ancillary services information into the EMR.   3.Fax lab results to patient's primary care physician and relay information to Team Manager and Hospice Care RN for follow-up.   4.Contact patient /family to verify equipment and/or supply delivery.  5.Process new patient admissions; existing patient discharges/transfers and deaths.   6.Prepare agenda for Interdisciplinary Team (IDT) Meetings and attend meetings to scribe.   7.Assist Team Manager and IDT members with various miscellaneous assignments.   8.Prepares patient and family for discharge and  follow-up care. 9.All responsibilities noted here are considered essential functions of the job under the Americans with Disabilities Act.
