We are seeking a dedicated and compassionate *PRN* Certified Registered Nurse Practitioner (CRNP) to join our hospice care team. In this role, you will be a vital member of the Interdisciplinary Care Team (IDT), working collaboratively with the Medical Director, physicians, and other healthcare professionals to provide exceptional, patient-centered hospice care. This position is ideal for a highly motivated and organized practitioner who thrives in a collaborative environment and is passionate about improving the quality of life for patients and their families. What You’ll Do As a CRNP in our hospice program, you will: Provide comprehensive medical care and assessments in both facility and home settings Perform physical examinations and preventive care under physician guidance Order, interpret, and evaluate diagnostic tests to assess patient conditions Develop and document treatment plans, findings, and prognoses Prescribe medications and recommend treatments as appropriate Participate in ongoing care coordination and case discussions Ensure compliance with Medicare Face-to-Face Encounter requirements What You Bring (Qualifications) Graduate of an accredited Nurse Practitioner program Current national certification as a CRNP Strong knowledge of hospice care principles and practices Familiarity with Medicare Face-to-Face Encounter criteria Excellent clinical assessment, diagnostic, and treatment skills Ability to work effectively within an interdisciplinary team Outstanding communication and organizational skills A genuine passion for compassionate, patient-focused care Why Join Us?
