Description Hospice Chaplain – Join Our Compassionate Care Team Are you passionate about providing comfort and spiritual support during life’s most meaningful moments? We’re looking for a dedicated Hospice Chaplain to join our interdisciplinary team and make a difference for patients and families when they need it most. What You’ll Do As a Hospice Chaplain, you’ll play a vital role in supporting patients and families through spiritual care and guidance. Your responsibilities will include: Conducting spiritual assessments and ensuring timely, appropriate care. Collaborating with the care team to develop individualized plans that meet spiritual needs. Providing direct spiritual care and emotional support to patients and caregivers. Serving as a liaison to community chaplains and spiritual counselors. Documenting services and maintaining records for quality assurance. Participating in interdisciplinary meetings and offering education on spiritual care. Recruiting and supporting spiritual care volunteers and community clergy. Leading memorial services and offering funeral support when requested. Contributing to quality improvement initiatives and team development. Experience working with death and dying, individuals/family/caregiver. At least 1 unit of CPE (Clinical Pastoral Education) Ability to work as member of interdisciplinary group. Comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs, and values. Ability to network with community clergy and congregations. Knowledge of and commitment to hospice philosophy of care. Hospice experience preferred. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
