We are the leading Hospice provider in the Roaring Fork Valley of Colorado, and we are searching for a Hospice Clinical Manager who shares our passion and who will focus on leading our dedicated team with a patient/family centered focus. This position requires someone with excellent clinical skills, who is a great communicator, and is skilled in fostering collaboration. Is responsible for Quality Improvement initiatives and spiritual and bereavement programs. Implements appropriate work structure and processes to accomplish goals for patient care specific to healthcare policy and procedures. Assists and supports clinical staff with the provision of patient care. Direct reports include as follows: Hospice RNs, MSWs, Chaplains, Occupational Therapists, Speech Therapists and Physical Therapists as needed, Volunteer Coordinator and Dietitian. Requirements o Two years’ experience in home health and/or hospice nursing o Management experience preferred o Possesses current license to practice as a registered professional nurse in the state of Colorado. o Adept at medical record documentation and Microsoft programs o Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. o Demonstrated ability to make meaningful observations and write comprehensive, accurate reports o Employment is contingent upon successfully passing a fingerprint-based criminal history background check through the Colorado Bureau of Investigation and CAPS check
