Providence Hospice is looking for a Hospice Clinical Supervisor (RN) for Full Time in Homer Glen, IL. The RN Hospice Clinical Supervisor leads our Interdisciplinary Team of RNs, Social Workers and Chaplains in providing care to patients and families. Providence Hospice provides quality patient centered care that ensures dignity, comfort and peace for our end-of-life patients and families. Education of staff regarding Medicare regulations and Conditions of Participation for Hospice. Training of new staff on computer software. Responsible for supervising RN Case Managers and home health aides. Management of clinical staff while keeping a high level of team spirit and expertise. Assist with determining and satisfying client requests. Review referrals and Plans of Care for completeness and accuracy. On-site assessments of staff for skills proficiency and infection prevention. Requirements Active Registered Nurse (RN) License for the state of Illinois. Hospice or Home health experience is required. 1-2 year(s) of management/supervisor experience. Knowledge of Hospice Philosophy of Care. Commitment to Clinical and Documentation excellence. EOE #HIGH1
