Job Purpose: As a CNA at Lifted Hospice, your primary function is to provide personal healthcare and related services in the patient's residence. You will assist in maintaining a safe and clean environment, work cooperatively with patients and their families, and operate under the supervision of a registered nurse. This opening is for a CNA to be housed in the field. Duties & Responsibilities: Provide direct patient care as directed by the RN, adhering to the CNA Plan of Care. Assist in patient transfers, ambulation, and maintaining a safe environment. Report and document changes in patient condition. Follow infection control practices and maintain a clean, safe environment. Uphold patient privacy and property, ensuring supportive and respectful communication. Assist with the patient's nutrition and hydration needs. Skills and Qualifications: High School Diploma or equivalent. State-approved CNA training program completion or state licensure program. Current CNA license and driver’s license required. Minimum one year in hospice, home health, or similar settings preferred. Effective communication and basic understanding of hospice care principles. Proficiency in using smartphones for communication. Reliable transportation and valid auto liability insurance. Working Conditions: Work in patients’ homes, long-term care facilities and nursing facilities in varied conditions including potential exposure to infectious diseases. The worker is subject to both environmental conditions: Activities occur inside and outside. The worker is exposed to infectious diseases and bodily fluids. Physical & Mental Requirements: Requires prolonged sitting and use of office equipment; may involve lifting, bending, and standing. Ability to lift up to 50 pounds and assist with patient transfers when necessary. Must be able to manage stressful situations calmly and professionally. Requires quick, sound decision-making in clinical and administrative contexts. Must work efficiently under deadlines and pressure to meet patient needs. Requires eye-hand coordination and manual dexterity.
