Summary of essential job functions Assists with all ADL's including bathing (Activities of Daily Living), Medication Reminders and Vitals, etc. Support and represent the mission of Providence as shown by your actions while on duty. Document changes in medical condition to the appropriate primary care nurse. Give and receive shift reports to ensure consistent medical care is delivered. Measure and record vital signs as necessary or instructed. Requirements Must be a CNA in the State of Illinois. Must have a compassionate, empathetic and nurturing personality. Home health and/or hospice experience strongly preferred. Comfortable traveling throughout the organizational geographic region. EOE
Responsibilities
The primary duty is to provide assigned clients with personal care and services according to the nurse case manager's hospice aide plan of care. This includes assisting with Activities of Daily Living, medication reminders, and recording vital signs.
