Azura Palliative Care and Hospice of WI, a brand-new hospice startup, is seeking an enthusiastic Hospice Community Liaison to connect our organization with the community and promote our compassionate care services. This is your chance to build meaningful partnerships, educate the community, and help us provide exceptional end-of-life care to those in need.

• Build Relationships: Forge and nurture strong connections with healthcare providers, referral sources (e.g., physicians, hospitals, nursing facilities, assisted living, and memory care communities), and community groups to generate referrals.
• Educate the Community: Promote Azura?s palliative and hospice care services by delivering presentations and providing information about benefits and access to healthcare professionals, patients, and families.
• Drive Outreach: Implement creative marketing strategies to meet patient volume goals and establish Azura as a trusted name in hospice care.
• Collaborate: Work closely with the hospice team, including nurses and social workers, to ensure seamless care coordination for patients and families.
• Track and Report: Maintain accurate records of referrals and interactions using designated CRM systems and monitor the cost-effectiveness of marketing efforts.
• Stay Informed: Keep a pulse on industry trends, competitor positioning, and community resources to support patients and families effectively.
