JOB SUMMARY AND QUALIFICATIONS

Responsible for the overall direction of the hospice services. Responsible for the delegation and coordination of hospice personnel evaluations. Establishes standards of care to comply with federal and state regulations and guidelines. Establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.

  • Ensures organizational compliance with legal, regulatory and accreditation requirements.
  • Evaluates hospice services and personnel using measurable outcomes and objectives.
  • Establishes and maintains effective channels of communication including integration or technology, as applicable.
  • Ensures that interdisciplinary care is provided, supportive services are available to personnel, and that appropriate service policies and procedures are developed and implemented to accomplish identified outcomes.
  • Ensures appropriate staff supervision during all service hours.
  • Monitors service utilization to ensure delivery of comprehensive care.

What qualifications you will need:

  • Ensures collaboration with agencies and vendors for effective management of services, and standards of ethical business and clinical practice are maintained.
  • A Bachelor’s Degree in a health care related field preferred. Three to five years experience in health care management.
  • This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
  • If a licensed healthcare provider, must be licensed in the state(s) where he/she will be providing services.

Hospice & Family Care provides physical, emotional, and spiritual support for patients and families. We help navigate each step of their hospice journey. Our Care teams include physicians, nurses, therapists, hospice aides and social workers. Also, music therapists, spiritual counselors, volunteers and bereavement specialists are part of the care team. Our services include regular scheduled visits and on-call support. Patients can receive hospice care at home or a skilled nursing center. Hospice care is also available at an independent or assisted living community or a hospital.
