We are seeking an LVN who will be responsible for providing direct patient care to hospice patients at our Hospice House in Redwood City. Schedule: Per diem; must be available at least 4 shifts per month Location: Redwood City Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to): Provides direct patient care under current nursing practice following a comprehensive assessment and the plan of care. Provide accurate and timely documentation of patient care services. Assess and provide patient and family/caregiver education and information pertinent to plan of care. Participates in the coordination of hospice services and report identified needs to the Program Director and interdisciplinary team. Education/Experience: Completion of a Licensed Vocational Nursing Education Program from an accredited school. Minimum of one year of LVN experience preferably in an acute care setting. Experience in a home health/ hospice setting preferred but not required Certificates, Licenses, Registrations: Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners. Valid California certification in Intravenous Therapy & Blood Withdrawal preferred Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car. Responsibilities
Provide direct patient care and support to hospice patients at the Hospice House in accordance with the established plan of care. Coordinate services with the interdisciplinary team and maintain accurate, timely documentation of all patient care activities.
