About the Role We are seeking a compassionate, relationship-driven Hospice Liaison to serve as the key connection between our hospice program, healthcare partners, referral sources, and the community. In this role, you will help ensure smooth care transitions, educate providers and families about hospice services, and support timely, appropriate referrals. The Hospice Liaison acts as the clinical and operational ambassador of our organization—building trust, and helping patients and families access compassionate, high-quality end of life care. Key Responsibilities Relationship Building & Outreach Develop and maintain strong partnerships with hospitals, skilled nursing facilities, clinics, and other referral sources. Meet daily with discharge planners and care teams to identify potential referrals. Educate providers and community partners about hospice philosophy, eligibility, and services. Care Coordination & Admissions Support Review clinical information to assess hospice eligibility; collaborate with Hospice Manager and clinical leadership. Facilitate smooth transitions from hospital or facility to home hospice, including coordinating documentation, signatures, and transportation. Ensure accuracy and completion of all admission documentation and referral processes. Assist with urgent situations and provide clinical guidance as needed. Business Development & Program Growth Identify new referral opportunities and emerging market needs. Contribute to referral and admission growth goals. Track referral metrics, identify trends, and share insights with leadership. Communication & Education Clearly and compassionately explain hospice services and benefits to patients, families, and caregivers. Provide ongoing education to healthcare teams about hospice processes—including general inpatient (GIP) services. Promote awareness of hospice services to increase understanding and accessibility within the community. Qualifications Associate of Science in Nursing (ASN) or higher degree required. Valid driver’s license and reliable transportation. Experience Minimum 3-5 years of experience in healthcare, preferably home health or hospice. Experience as a charge nurse, preceptor, or in a leadership/support role preferred. Skills Strong communication, presentation, and relationship-building skills. Excellent customer service and problem-solving abilities. Ability to work independently and manage multiple priorities. Proficiency with Microsoft Office and related software. Understanding of Medicare Conditions of Participation (OASIS/HOPE experience a plus, or willingness to learn). Compassion, empathy, adaptability, and professionalism in emotionally sensitive situations. Working Conditions This is a traveling position; reliable transportation is required for varied weather conditions. Physical Requirements Ability to walk or stand for prolonged periods.
