Key Responsibilities Provide skilled nursing care to hospice patients under the supervision of an RN, following the plan of care. Monitor, document, and report changes in patient condition — including physical, emotional, and psychosocial needs. Administer medications and treatments according to physician orders and agency policies. Support families with education, clear communication, and a reassuring presence during emotionally intense moments. Collaborate closely with the RN Case Manager and the interdisciplinary team to ensure coordinated, patient-centered care. Maintain accurate, timely electronic documentation that meets CareM standards. Participate in team meetings, case conferences, and quality improvement efforts. Requirements What We’re Looking For Current Texas LVN license in good standing. Graduate of an accredited vocational nursing program. Hospice experience preferred; home health or acute care experience also valuable. CPR (BLS) certification. Strong communication skills and the ability to remain calm, steady, and organized under pressure. Comfort working independently in the field while staying connected to a supportive team. Reliable transportation, valid driver’s license, and current auto insurance. Proficiency with EMR systems and basic technology.
