The LPN responds to patient needs, provides symptom management, and offers support to patients and families during end-of-life care. Responsibilities Provide direct patient care to hospice patients in their homes or hospice facilities. Conduct comprehensive assessments of patient needs, including physical, emotional, and spiritual aspects. Administer medications, treatments, and interventions as prescribed by the hospice interdisciplinary team. Monitor patient vital signs, symptoms, and overall condition, reporting any changes or concerns promptly. Communicate effectively with the hospice interdisciplinary team, including physicians, registered nurses, social workers, and chaplains. Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment. Document all patient care activities accurately and thoroughly in the electronic medical record. Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care. Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients. Participate in ongoing education and training to maintain licensure and enhance clinical skills. Requirements Education: Graduation from an accredited Licensed Practical Nurse program. Licensure: Current LPN license in the state of practice. Current Driver’s License. Experience: Minimum of one year of nursing experience, preferably in hospice or palliative care. Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Transportation: Reliable transportation and valid auto liability insurance.
