Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest. Job Responsibilities: Good Shepherd Community Care is pleased to offer this opportunity for a highly skilled Licensed Practical Nurse seeking to practice at the top of their license.The Hospice LPN will provide the highest quality nursing care for all hospice patients following the established plans of care. The ideal candidate understands the importance of providing an exceptional level of nursing services and compassionate end-of-life care for clients. This LPN must be professional, reliable and organized, and must demonstrate effective communication skills. Hospice LPNS perform the full range of their professional capabilities. LPNs with a growth mindset and a collaborative approach flourish at Good Shepherd. Must have demonstrated skills in patient/family evaluations, pain management, symptom control, and communication. Schedule and Weekly Hours: Monday to Friday, from 9 am to 5 pm. Education, Skills, and Experience: Licensed Practical Nurse (MA license) A minimum of two years prior experience in community health, long term care facility, oncology, or a related field preferred. Ability to be a team player and work harmoniously with patients, families, co-workers, and management. Providing care consistent with written instructions. Providing assessments of the impact of the terminal diagnosis on the patient’s physical, functional and environmental needs and activities of daily living. Providing on-going evaluation of patient/family response to care and informing the Nursing Care Coordinator and other personnel of changes in the patient’s needs and outcomes of interventions. Preparing documentation that demonstrates adherence to the patient’s individualized plan of care and progress towards established goals. Assessing for patients’ safety. Providing personal care including assistance with daily hygiene, ambulation, exercise, toileting, nutritional support, dressing changes and other care as designated by the Nursing Care Coordinator. Assisting with medications that are ordinarily self-administered. Assisting patients with household activities.
