Your Day-to-Day: Conduct in-depth patient assessments, diving deep into physical, emotional, and holistic needs in the Greensburg, Waynesburg, and Washington areas. Build tailored care plans that prioritize the voice and wishes of our patients and their families. Be the crucial link, communicating vital updates between physicians, patients, and caregivers. Experience: At least 3 years as a Licensed Practical Nurse (LPN), with 1 year in Hospice Care. Licensure: Current State Board of Nursing License (LPN) in Pennsylvania & valid CPR certification.
