Job Title: Hospice MSW Job Schedule: PRN, paid per visit Job Summary: The Medical Social Worker will provide psychosocial assessments and counseling services to hospice patients and their families under the direction of the attending physician. This role involves collaborating with the Interdisciplinary Group/Team (IDG/T) to coordinate care and ensure that patients receive holistic, compassionate support tailored to their needs. Key Responsibilities: Conduct psychosocial assessments to evaluate the emotional, social, and environmental factors impacting the patient and family. Provide emotional support and counseling to patients and families coping with end-of-life issues. Collaborating with the hospice interdisciplinary team, including nurses, social workers, and other healthcare professionals, to provide holistic care. Participating in team meetings to discuss patient care plans. Assist families in understanding and managing grief and loss. Help families access community resources and services. Maintain documentation of social work services in accordance with hospice policies and regulatory requirements. Requirements Education: Social Work (BSW or MSW) degree from a school of social work accredited by the Council on Social Work Education; or Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or Baccalaureate degree in psychology or sociology and is supervised by a MSW; and has 1 (one) year of social work experience in a healthcare setting; or Baccalaureate degree from a school of social work accredited by the Council on Social Work Education. Licensure: Licensed as required by the state where practicing. Current driver's license. Experience: One (1) year of experience in palliative care or hospice preferred. Skills: Working knowledge of community resources. Good interpersonal skills. Demonstrates ability to work with computers. Transportation: Reliable transportation and valid auto liability insurance.
