An Excellent Opportunity Mountain Valley has an opening for a Registered Nurse Liaison who will work in the Winston-Salem service areas.  As a hospice liaison you will have the responsibility for responding to hospice referrals, preparing with our care teams for services needed, and providing a physical assessment and admission to hospice as required.   Duties * Maintain a hospice presence in the area hospitals * Acts as a partner in care with physicians, case managers and other facility staff.  * Educate patients and families about the hospice benefit and services.  * Provides an initial physical assessment and admission to hospice as required.  * Initiate the referral to admission process through gathering information, education, and form completion aspects of patient intake, admissions, and transition to home. * Perform hospice in place (hospital) admissions as needed.
