This critical role ensures that patients receive expert care and guidance during off-hours, helping to manage symptoms, provide comfort, and support families through the challenges of end-of-life care. Assess patient conditions, manage symptoms, and provide necessary nursing interventions. Provide guidance and support to patients' families and caregivers over the phone or in person as needed. Collaborate with the hospice interdisciplinary team to ensure continuity of care. Document all patient interactions, assessments, and interventions accurately and timely. Adhere to hospice philosophy and policies in all aspects of care delivery. Admit patients who are hospice appropriate and interact with families in assessing needs and treatment plan.
