Job Details Job Location: Farmington Hills Hospice - Farmington Hills, MI 48334 Patient Services Coordinator (PSC) Be the operational hub that keeps hospice visits coordinated, teams aligned, and patient care moving without delays. The Patient Services Coordinator supports hospice operations by coordinating visit scheduling and ensuring clear, timely communication between clinical staff and leadership to support consistent patient care delivery. Microsoft computer skill set required, experience with The Care Teams software and applications preferred. Must be computer literate and ability to maintain simple records in English. Excellent oral, written and analytical skills. Should be a skilled organizer able to manage office files and logbooks. Must possess light clerical skills and have a polite telephone manner.
Responsibilities
The Patient Services Coordinator assigns evaluations, admissions, and routine visits while processing rescheduled and missed visits. They ensure timely communication between clinical staff and leadership to support consistent patient care delivery.
